課程大綱Course Description：

本課程旨在於協助學生獲得長期照護個案管理的基本知能，包含個案管理的概念和理論、重要性、個案管理者的角色、功能和職責。學生在修課結束後，能夠瞭解個案管理的流程和模式，具備長期照護個案管理及實用的專業能力。

English Outline：

This course aims to provide basic knowledge and skills in long term care case management, including the theories and concepts of case management, the importance case management, and the roles, functions and duties of the case managers. Upon completing the course, students will be able to understand procedure and models of case management. Besides, students will acquire the professional skills and practical applications of case management in long term care.
